Doctors attending medical congress session on endocrinology and therapy

🫀Modern therapy in Transbaikalia: from traditions to innovations Today, April 9, the XIV Congress of Therapists of the Trans-Baikal Territory began its two-day work at the Chelyabinsk State Medical Academy. This is an annual large-scale event of the professional community. Among the participants were specialists from other regions of Russia. Acting Rector of the Chelyabinsk State Medical Academy Natalia Laryova, opening the plenary session, emphasized the value of the therapeutic school of Transbaikalia. She emphasized the need to strengthen preventive work, dispensary observation of patients with chronic diseases, a harmonious combination of classical experience with innovations, the fight against risk factors and the formation of a responsible attitude to the patient's health. The main goal, as before, is to provide qualified medical care to Transbaikalians, tireless care for strengthening their health. This ran like a red thread through the speeches of the Honorary Rector of the Chelyabinsk State Medical Academy, Doctor of Medical Sciences, Professor, Honored Doctor of the Russian Federation Anatoly Govorin; Head of the Department of Therapy of the Faculty of Additional Professional Education of the Chelyabinsk State Medical Academy, Doctor of Medical Sciences, Professor, Honored Doctor of the Russian Federation Natalia Lareva; Head of the Department of Hospital Therapy and Endocrinology of the Chelyabinsk State Medical Academy, Doctor of Medical Sciences, Professor Olga Serebryakova; Professor of the Department of Therapy of the Irkutsk State Medical Academy of Postgraduate Education Natalia Khramtsova. The program of the congress includes master classes, plenary and breakout sessions, a competition for young therapists. The task of the congress participants is to determine the priority areas for the development of the region's therapeutic service in the context of the national project "Long and Active Life", to exchange best practices in patient management and to discuss modern approaches to the prevention of chronic diseases, taking into account the regional characteristics of their course.📝🏥

👥 The format of the congress of therapists includes more than 20 master classes, interactive discussions, plenary and breakout sessions. Experienced and novice doctors will discuss about 80 interdisciplinary medical topics. At a clinical lecture on the role of vaccination against pneumococcal infection, an important and often underestimated question was raised: how vaccination affects the life expectancy of patients. The speaker was Professor of the Department of Therapy of the Irkutsk State Medical Academy of Postgraduate Education, Chief Freelance Cardiologist of the Ministry of Health of the Irkutsk Region, Doctor of Medical Sciences Natalia Khramtsova. The expert stressed that timely vaccination is not only protection against infection, but also a real tool to reduce the risk of severe complications and mortality, especially in comorbid patients, the elderly, as well as patients suffering from COPD. cardiovascular diseases and concomitant pathology. It is for this category that even a seemingly "ordinary" infection can pose a serious threat. Natalia Khramtsova noted that vaccination remains the only effective way to influence the incidence and mortality from pneumococcal infection. According to her, over the past 50 years, it has saved more than 150 million lives around the world. The importance of immunoprophylaxis was especially evident during the COVID-19 pandemic. During the lecture, the mechanisms of the influence of pneumococcal infection on the course of cardiovascular diseases and the relationship between infectious complications and an increase in the risk of adverse outcomes were also considered.💉

🩺 "A properly compiled medical history is 80% of successful treatment." This is one of the main conclusions voiced during the competition of young therapists within the framework of the XIV Congress of Therapists of the Trans-Baikal Territory. And, judging by the presented works of young doctors, this success is obvious. Difficult cases of clinical practice, prompt actions, sometimes making extraordinary decisions in the name of saving the patient's life - all this united the speeches of young therapists. Clear answers were given to the questions of the Acting Rector of the Chelyabinsk State Medical Academy Natalia Lareva and other members of the jury under her leadership.

"The participants not only presented the anamnesis and results of the examination, but also justified in detail the chosen treatment tactics with a deep understanding of the pathogenesis of diseases and knowledge of modern therapeutic approaches," commented Dmitry Serkin, head of the scientific department of the Academy, a member of the jury.

💫

As the competition showed, Chelyabinsk State Medical Academy graduates are ready to solve complex clinical problems, think critically and not get lost in difficult circumstances.

"How could it be otherwise, because the teachers of our beloved academy not only give solid fundamental knowledge, but also teach how to apply it in real clinical conditions, forming real professionals from students," says Radnakhanda Tsybendorzhina, endocrinologist of the endocrinology department of the Trans-Baikal State Regional Hospital.

- I am sincerely happy for you, dear young colleagues, the future of medicine in Transbaikalia is in good hands! - said the acting rector of the Chelyabinsk State Medical Academy Natalia Laryova, presenting the winners of the competition with well-deserved awards. The diploma for the first place was awarded to the assistant of the Department of Outpatient Therapy with a course of medical rehabilitation of the Chelyabinsk State Medical Academy, cardiologist Andrey Miroshnikov, resident of the Department of Infectious Diseases of the Chelyabinsk State Medical Academy Yulia Nepokrytova, cardiologist of the clinical medical center Kristina Mityukova. The diploma for the second place was awarded to residents of the Department of Therapy of the Chelyabinsk State Medical Academy Irina Sverkunova and Vladimir Porotnikov. 🍏 Obesity, thrombosis and diabetes: what therapists and endocrinologists talked about at the Congress at the Chelyabinsk State Medical Academy One of the sites of the Congress of Therapists is the section "Dialogue between a therapist and an endocrinologist". It united doctors around topics that have long gone beyond narrow specialization. Transbaikal doctors - employees of the Department of Hospital Therapy and Endocrinology of the Academy - talked about how hormonal disorders affect the heart and blood vessels, why it is important not to miss the early complications of diabetes and how to recognize hidden risks in time - from metabolic failures to thrombosis. The experts presented understandable and practical algorithms that help identify threats to life and health at an early stage. Special attention was paid to obesity and its negative impact on health, especially in women and children.

"A child's health begins with the health of the mother: what the mother eats during pregnancy and what kind of lifestyle she leads, through epigenetic mechanisms - in particular, DNA methylation and changes in gene expression - can affect the work of genes and manifest itself in the next generation or even in a generation," said Dmitry Serkin, Associate Professor of the Department of Hospital Therapy and Endocrinology of the Chelyabinsk State Medical Academy, Candidate of Medical Sciences, Associate Professor.

According to him, one of the modern hypotheses for the growth of obesity and type 2 diabetes mellitus is the impact of hunger in previous generations. For example, the famine experienced by grandparents during the world wars could affect the health of grandchildren and great-grandchildren through epigenetic changes, contributing to the formation of insulin resistance as an adaptation mechanism to food deficiency. The final topic of discussion was the rehabilitation of patients with thyroid diseases. According to experts, therapeutic exercise and physiotherapy are an important stage of recovery, which directly affects their quality of life.
